Appointments
Call us on Monday through Thursday between 8:00 am to 5:30 pm and
on Fridays from 8:00 am to 4:30 pm to schedule your appointment. Appointments
and surgery can be scheduled on any day of the week. If you are unable
to keep your scheduled appointment time, please give 24 hour notice.
You
may download a copy of the PDF or RTF file of the initial visit
client questionnaire that will need to be completed upon your arrival.

file and have it completed prior to your first appointment in order
to save time.

Before
You Come
The Ophthalmology Service may need certain medical records from
your veterinarian. Your veterinarian may send them to us, or you
may be asked to bring them with you on your first visit.
Fees
The fees for service are usual and customary for the specialized
service rendered. Before we schedule significant procedures, you
will be given a written estimate of the anticipated costs. We will
require a deposit of 50% before the procedure. The balance will
be due when you pick up your pet. Payment is expected at the time
of service. We accept the following forms of payment: cash, MasterCard,
Visa, Discover, Care Credit and checks are approved through Telechek®.

Try to arrive 15 minutes before the scheduled appointment time.

Please use and bring all medications with you that your veterinarian
has prescribed. Please bring with you or have your veterinarian
FAX any laboratory tests that have been performed. The FAX number

When bringing a pet in for surgery, they are to be fasted from
8:00 p.m. the previous evening, however, they can have water
up until the time of drop off for the surgery. |

On your first visit, Dr. Priehs or Dr. Denis will evaluate your
pet and go over all of the pertinent ophthalmologic findings
with you. Other special diagnostic tests and procedures will
be discussed at that time. Our appointments generally run on
time with the exceptions of emergencies. Generally your first
office visit will run anywhere from 15 minutes to 30 minutes
depending on the evaluation. |
